Order Form Self-Mailer with Envelope
This order form has a weld-seam pocket with remoistenable glue stripe and perforation for easy removal of the envelope. This self-mailer was further fugitive spot glued by Glue-Fold to seal the piece to US Postal Regulations for bulk mailing.
